Positions' Responsibilities and Qualifications
Group Home Manager
-
Group Home Supervisor job duties include supervising everyday operations as well as staff in an assigned group home.

In charge of writing and implementing ISP plans and behavior-building plans and presenting these plans to staff as necessary.

Oversee family and/or guardians of the clients while maintaining assigned group homes in accordance with state licensure standards.

Supervisors also must be able to monitor and record individual clients' progress using SyAngi record-keeping system for data collection.

Assisting home members with their personal care along with attending medical appointments.

High school diploma or equivalent
-
1-2 years of experience in a management position preferred.
Direct Care Provider

Ensure the highest quality of client safety and care for the residential individuals having developmental disabilities in the home and community.

Assist members in performing activities of day-to-day living
-
Support the individual's medical management
-
Provide teaching and support for individual treatment plans
-
Maintain thorough program and behavior documentation of teaching programs, incident reports, and activities.
-
Maintain program documentation, safety, implementation, and cleanliness in the home.
-
Partake in necessary training sessions as requested by the home supervisor
-
Maintain good relations with co-workers, clients, guardians, and public peers.

High school diploma or equivalent
CNA, LPN, Certified CareGiver

Implementation of the care plan.

Maintain accurate and complete patient documentation.

Provide care that promotes continuity, quality, and member safety
-
Provide skilled nursing care to members in the home.

Medication distribution
-
Medication reorders as needed (or communicating with the home manager that a medication needs to be reordered)

Maintain medical equipment and supplies, and checks equipment apparatus or appliances furnished to residents in the course of treatment

Assist members with comfort, grooming, and hygiene needs as needed.

Diet management
-
Specimen collection.

Collaborate with the Nurse Director, Medical Group Home Manager, and care team for reporting all medical aspects of care and changes in conditions regarding the members in the home.

Ensure that residents are being emotionally and physically stimulated to the best of their ability while evaluating patient progress toward their goals on a day to day basis.
